COMMITMENT FORM(For practitioner)

My name: LÊ ANH TÚ; Dharma name: MINH TUỆ; Date of birth: 19/05/1981; Nationality: Vietnam; Passport No.: [redacted] Date of issue: 07/11/2024 Date of expiry: 07/11/2034

I am currently practicing the Dhutanga (Austere Practice) – wearing robes made of discarded cloth, keeping only 3 robes and 1 alms bowl, regularly observing the following ascetic practices:

• Going for alms and eating only once a day in mindfulness;

• Not accepting food after that time;

• Resting under the shade of trees, in deserted houses, in the forest, in graveyards, in open fields;

• Not lying down to sleep;

• Keeping the monastic precepts as taught in the Buddhist scriptures.

During the course of my practice, I commit to the following:

1. My practice is entirely voluntary, with the consent of my parents, spouse (if any). I am under no coercion, inducement, nor involved in any economic, political, or religious agenda. I take full responsibility for my own health, safety, and compliance with the law, and do not request intervention or assistance from my family unless specifically asked.

2. I am not involved in any activities that violate the laws of any nation, am not wanted or prosecuted, have no criminal record, and do not discriminate against any group based on gender, race, or occupation.

3. I take full responsibility for my words and actions, whether in front of the public or anyone around me. I do not represent any individual, organization, or group in making public statements or appearing in the media.

4. My words, actions, and demeanor are my own responsibility. I do not represent or participate in the establishment of any religious group, sect, or association.

5. I do not accept money, property, land, or any material goods unrelated to my ascetic practice. Any items I receive, such as food, drink, shelter, transportation, paper, or necessities, are offered voluntarily by individuals or organizations without solicitation. I bear legal responsibility for my use of such offerings, ensuring they are for my personal basic needs only. If any item poses danger to my life or well-being, I will stop using it and take responsibility for returning or relinquishing it.

6. I do not solicit anyone to join me in practice or to follow me. If there is any individual or group claiming to do so, they are acting on their own responsibility. I am accountable before the law of the host country for my words and actions.

I hereby declare that I fully understand, agree, and commit to comply with the laws of the host country and the content stated above.

• उसके बाद भोजन स्वीकार नहीं करना;

• वृक्षों की छाया, परित्यक्त घर, वन, श्मशान या खुले मैदान में विश्राम;

• लेटकर नहीं सोना;

• बौद्ध धर्मग्रंथों में बताए गए सभी विनय नियमों का पालन।

अपने साधना काल के दौरान, मैं निम्नलिखित बातों के लिए प्रतिबद्ध हूँ:

1. मेरा साधना अभ्यास पूर्णतः स्वेच्छा से है, माता-पिता, जीवनसाथी (यदि कोई हो) की सहमति से। मुझ पर कोई दबाव, प्रलोभन नहीं है और न ही मेरा किसी आर्थिक, राजनीतिक या धार्मिक उद्देश्य से संबंध है। मैं अपने स्वास्थ्य, सुरक्षा और कानून के अनुपालन की पूरी जिम्मेदारी लेता/लेती हूँ और परिवार से किसी हस्तक्षेप की अपेक्षा नहीं करता/करती जब तक कि मैं स्वयं न कहूँ।

2. मैं किसी भी राष्ट्र के कानून का उल्लंघन करने वाली गतिविधियों में संलग्न नहीं हूँ, न ही किसी मुकदमे में अभियुक्त हूँ, न ही मेरा कोई आपराधिक रिकॉर्ड है। मैं लिंग, जाति या पेशे के आधार पर किसी के प्रति भेदभाव नहीं करता/करती।
